Join us for a virtual event designed to energize, educate, and engage caregivers and colleagues in Providence’s journey to becoming an Age-Friendly Health System.

To better serve our communities, Providence is one of five pioneering health systems to participate in the creation of an Age-Friendly Health System, which is based on the 4Ms framework: What Matters, Medication, Mentation, and Mobility. In 2019, Providence’s Clinical Council added an essential 5th M for Malnutrition, another common issue for older adults.

During this 3-hour symposium, pioneering partners Don Berwick, MD and Amy Berman, RN will join Providence leaders to discuss the impacts of COVID-19 on older adults, Providence’s commitment to leading as an Age-Friendly Health System, and how we can leverage our “5Ms” framework to guide our way.

At the conclusion of this presentation, the attendee will provide better patient care through an increased ability to review the impact of COVID-19 on older adults and explore the evidence that age-friendly health systems positively impact care of the older adult.

10 a.m. - Age-Friendly Care at Providence: Care Across the Continuum

At the conclusion of this presentation, the attendee will provide better patient care through an increased ability to review and discuss caring for older adults across the care continuum and examine the application of AFHS principles in a real-world example (e.g., in the outpatient setting, emergency care setting, acute/inpatient setting, and home and community care setting). The attendee will also have an increased ability to outline the essential elements to AFHS designation and formulate clinical rationale that supports AFHS designation.
